Nursing Services
Depending on assessed need and authorization, nursing may include comprehensive or focused assessments, development and review of nursing plans, monitoring health status, nursing interventions, medication-related support within scope, delegated tasks, staff and caregiver education, communication with healthcare professionals, and follow-up when health needs change.
Professional Therapies and Supports
Physical Therapy
Mobility, strength, balance, gait, transfers, positioning, range of motion, fall prevention, and functional movement.
Occupational Therapy
Daily living skills, upper-extremity function, fine motor skills, cognition, sensory needs, adaptive equipment, and environmental access.
Speech-Language Pathology
Communication, language, cognition, voice, swallowing, mealtime safety, and augmentative or alternative communication needs.
Behavioral Support
Assessment, behavior support planning, prevention strategies, skill building, staff or caregiver training, data review, and rights-respecting intervention.
Dietary Services
Nutrition assessment, special dietary needs, meal planning, education, and coordination around health and swallowing considerations.

Assessment and support related to hearing, communication access, and recommended hearing devices or strategies.
HCS Social Work
Authorized HCS social work focused on psychosocial needs, adjustment, relationships, resources, and coordinated support.
HCS Cognitive Rehabilitation
Authorized HCS support for functional cognitive skills following an acquired brain injury or other qualifying need.
Telehealth When Allowed and Appropriate.
Certain professional services may be delivered using secure synchronous audio-video technology when program rules allow it, the individual or legally authorized representative consents, the service is clinically appropriate, privacy requirements are met, and the authorized plan supports that delivery method. Telehealth is not promised for every service or person.
