The Home and Community-based Services program helps eligible Texans with intellectual disabilities or related conditions receive services in community settings rather than institutional settings.
As an HCS program provider, United Hands works with each individual, legally authorized representative, service coordinator, and service planning team to deliver authorized supports connected to the person-directed plan and individual plan of care.
The Plan Belongs to the Person.
HCS services are not selected from a generic package. The individual’s preferences, assessed needs, desired outcomes, authorized plan, health and safety, and chosen living arrangement shape what support looks like.
HCS Services Available Through United Hands
Every authorized service has a purpose: greater independence, stronger health and safety, meaningful
relationships, community belonging, and progress toward the life the individual chooses.
A Home Should Feel Like Home.
When an HCS residential option is authorized and available, United Hands focuses on privacy, choice, relationships, safety, daily routines, community access, and the individual’s control over ordinary life.
Residential placement is never automatic. It depends on the person’s authorized plan, informed choice, compatibility, setting readiness, staffing, health and safety needs, and United Hands’ ability to meet those needs.
Understanding HCS Eligibility
HCS eligibility and enrollment are determined through Texas Health and Human Services and the applicable local intellectual and developmental disability authority—not by United Hands.
A person must meet program, level-of-care, financial, and other requirements and have an available waiver opportunity. United Hands can explain its provider services and participate in the provider selection and service-planning process, but cannot promise eligibility or a waiver slot.
