Community First Choice Personal Assistance Services/Habilitation combines hands-on assistance with opportunities to learn and retain daily living skills.
United Hands supports eligible individuals according to the person-centered service plan—providing respectful assistance while encouraging the greatest level of participation and independence that is safe and meaningful for the person.
CFC PAS/HAB May Address
- Eating, toileting, grooming, dressing, bathing, and transferring
- Meal planning and preparation
- Household tasks needed for a clean, sanitary, and safe home
- Mobility and use of adaptive equipment
- Shopping, money management, and other independent living activities
- Communication, personal decision-making, and community participation
- Escort assistance to access services or activities, as allowed by the plan
- Authorized health-related tasks, including delegated or assigned activities when permitted
- Skill building that helps the individual acquire, retain, or improve self-help and daily living abilities
Important Transportation Distinction:
CFC Escort: Accompanying and assisting an individual to access services or participate in community activities.
CFC escort does not itself include transporting the individual. Transportation must be authorized and delivered under the appropriate program service or separately arranged.
