If You Already Receive HCS or TxHmL
- Contact United Hands and tell us which program and services you are exploring.
- Tell your LIDDA service coordinator that you want information about United Hands.
- Participate in a conversation, tour, or provider meeting to learn about our approach and current capacity.
- If you choose United Hands and the service planning team agrees on the next steps, authorized records and plans are coordinated through the proper process.
- United Hands reviews needs, staffing, setting readiness, qualifications, risks, and capacity before accepting services.
- Services begin only after required authorizations, plans, consents, provider arrangements, and transition steps are complete.
If You Are Not Yet Enrolled
- Contact your local intellectual and developmental disability authority for information, screening, service coordination, and applicable waiver interest-list or enrollment processes.
- Eligibility and waiver opportunities are determined by Texas Health and Human Services and the appropriate local authority—not by United Hands.
- United Hands can describe our services and help you prepare questions, but we cannot place someone into HCS or TxHmL, guarantee Medicaid eligibility, or promise a waiver opportunity.

- Name and preferred contact information
- Whether the person is enrolled in HCS, TxHmL, another program, or is not yet enrolled
- Name and contact information for the LIDDA service coordinator, if applicable
- Services being requested and preferred start timeframe
- General location and transportation needs
- Accessibility, communication, staffing, medical, behavioral, or safety considerations that affect provider capacity
